Eligibility Process How the Process Works1) Request Accommodations and Submit documentation Complete the Accommodation Request form. Attach your documentation to the Accommodation Request Form. Types of Documentation: Documentation may include assessments, reports, or letters from qualified evaluators, professionals, or institutions such as IEP, 504 Plan, or Psychological Evaluation. If Documentation submitted is insufficient you will be contacted within 10 days and asked to submit a Disability Verifcation Form completed by a healthcare provider.2) Intake MeetingThe purpose of the intake meeting is to learn from the student, what functional limitations they are experiencing and what accommodations they are requesting. The Accessibility Services Coordinator will contact the student to schedule an intake meeting once forms have been submitted. This interactive process allows us to work together to determine the most reasonable accommodations for the student. 3) Evaluation of EligibilityAccessibility Coordinator will evaluate all materials, consulting with appropriate offices to determine reasonable accommodations. 4) Student Notification of Accommodations Students will be notified by email within 10 working days of the intake meeting.Important DetailsDeadlines Matter! Submit requests on time to ensure accommodations can be arranged for the upcoming semester. Requests submitted after the deadline cannot be guaranteed for the current semester.Accommodations cannot be applied retroactively; therefore, this entire eligibility process must be completed prior the implementation of requested accommodations.
